Judy,

I think you are on the right track. I would suggest she trigger her MER
by a little fingertip expression a full 5 minutes before offering the
breast so that the "big push" has come and gone.

Smaller feedings oftener would give smaller "doses" of lactose at one
time for the baby's lactase supply to deal with. Someone pointed out to

I have been having success encouraging mothers to gradually "work their
way up" to all feedings in a 4-6 to 8-10 hours (the operative word here
is "gradually" so as to avoid plugged ducts, mastitis, etc. on the other
side) even to as much as all feedings in a 12 hour period on the same
breast, then all feedings for the next 12 on the other. It's important to
take it slowly, and I think you would see changes in the baby's behavior
within a day or two.

Someone told me that in his book, Dr. Newman even goes so far as to have
certain mothers nurse 24 hours on the same side, and change to the
opposite breast for the next 24 hours.
