I don't think it can be the full story by any means.
The abstract tells us that the sample was of 151 mothers and their 176
preterm infants (gestational age at birth <32 weeks or birthweight <1500 g).
Of these, 73 mothers were sero-positive for CMV (member of the herpes family
of viruses). 27 of these mothers (37%) transmitted the virus to their (33)
babies, of whom only 4  showed symptoms serious enough to be termed
"sepsis-like" by the authors. The authors don't tell us what happened to
these four babies, at least, not in the abstract. Half of the 33 babies
infected with CMV them showed no symptoms at all.
So, what does this tell us? Simply, that some very tiny pre-term infants
born of some CMV infected women whose breastmilk transmits the virus to
them, stand a small risk of getting sepsis-like symptoms as a result of
being breastfed.
I wonder who paid for the research.
