Where I work all the midwives are expected to know how to resuscitate an
infant at birth, but we don't intubate; it happens so seldom around here
that even the pediatricians don't do it more than a couple times a year
each.  We have about 2000 births/year, which, BTW, makes us a 'large'
hospital in Norway.  We are the regional center for neonatology and have
babies down to about 28 weeks in our NICU.
Other nursing care staff are expected to know how to do infant CPR.  In the
15 years I have worked here I have never, never, not even once, had to
resuscitate a baby on postpartum or in the outpatient clinic, which has only
existed for 10 years.  I don't think anyone else here has had to either, in
that time.  It would be a huge sensation if it happened.  We had a baby die
in about 1991 of undiagnosed sepsis on the second day of life.  That baby
was found lifeless in its bed; CPR would have made no difference there.  We
all remember it with horror.  There have been no other unexpected deaths.
I guess there have to be a lot more than 150 births/month before you start
routinely coming across babies who stop breathing with no warning.
