I was interested in the post from Ukraine about the mom with the chronic
cracked nipples.  I would suspect that the mother has some sort of skin
condition that predisposes her skin to breakdown.  Broken skin is easily
infected by the pathogens in the baby's mouth.  A key to helping this mother
lies in her experience at the sea shore.  It appears that when she is
rinsing off her skin frequently in salt water, her nipples heal.  I suspect
that the constant washing with a saline solution (as in sea bathing) keeps
the colonies of bacteria reduced to the point that her skin can heal.  Why
not suggest that she make up a bowl of warm water with a pinch of salt in it
and dip and rinse her nipples after breastfeeding?  She could do this along
with maybe several days of applying a mild antibiotic cream
(over-the-counter should be sufficient.)  A dermatologist might be useful,
because if she has eczema (as the primary cause for the chronic skin
breakdown) that can be treated.
