The problem probably originates with the pacifier :-(  Babies can coast along fairly well on mom's original burst  of milk supply, but not enough sucking early on leads to faltering supply as you are now seeing -usually anywhere from 1-3 mo.  Formula gets introduced and the next thing you know, baby is formula fed and BF is over :-(.  (My opinion).  
Baby A probably needs to be treated for thrush, in spite of Dr's opinion and mom needs to be treated again at same time.  Baby B needs rule 1 - Feed the Baby.  However you can, with whatever you can get (mom's milk or formula).  Supply in both moms needs to be stimulated with frequent nursing and pumping.  Pat in SNJ
