Here's another "good as any" guess:

When she's feeling "PMS-y", the mom's cooking habits change.  Either she's
too cranky to cook the way she usually does, or certain cravings send her in
certain directions.  As a result, maybe she's serving up more prepared foods
with more additives, or maybe she's leaning more heavily on dairy or citrus
or some such.  The baby, *eating those foods directly* (as well as getting
them through her milk), responds with stuffy eustachian tubes, and being
asymmetrical like the rest of us, feels it more in one ear than the other
and begins to tilt.  By the time the baby is downright sick, the mom is over
the hump and everything clears.

Hey, it's as good as anything hormonal I could think of.
