This is almost certainly the case. I can think of only one article I've 
written for which I can claim responsibility for the title. At newspapers, 
magazines, and websites, one of an editor's jobs is to create titles and 
captions. IME, a writer may or may not be given a final proof of the 
article to review and more often than not only sees the titles, subtitles, 
and captions after publication. Sometimes with great amazement!
