Given that nipple blebs have been related to excessively strong pumping or
perhaps even unusually strong infant suckling, could it be possible that the
forceful ejection of milk through the ducts could possibly damage those
ducts, causing milk to break out and through negative pressure rising to the
nipple surface looking for an exit? 

Could the skin growing over the milk bleb in fact be considered a new ductal
route trying to create a new nipple opening, and just not breaking through? 
Just more ruminating...
