Lois,
We make follow-up phone calls at 2-3 days post discharge.  We use the
reverse side of the sheets used to make our notes when we see moms
during their hospital stay.

We ask about # of voids and stools/day, how often infant is eating,
audible swallowing, if supplementing, nipple soreness, breast fullness
and softening after feeding.

These questions give us a pretty good idea how things are going.  If we
are concerned we usually ask if we can call back the next day.
Sometimes we encourage the mom to come in so we can help with specific
problems.

We also ask if we can call again in a couple of weeks.  At that time we
discuss growth spurts.  It also gives us an idea how many of our moms
are still breastfeeding at 2 weeks.  I use this info for our statistics.

We use a filing system similar to what fellow- Lactnetter Sharon Knorr
discribed.  Very simple but I think efficient.
