I have asked moms to obtain a prescription for domperidone when there is no prolactin surge. My thinking was that if we raise the base level to closer to surge level then there is more prolactin to do the work. I have had some success with this, and mothers have found milk supply does increase. Note, no formal studies, just a couple of experiences in the clinic where I work. I would say it is worth a try with this mom if she is willing to go on the medication. This would be an interesting study for sure!
