Hello,
  I'm counseling a G3, P3 mom that complained of rather sudden onset of
right nipple pain while the baby is nursing.  Baby is about 6 weeks old and
has nursed well since birth.  Mom had no initial nipple pain and says she
has never felt this kind of pain before--she nursed her previous two
children.  She denies stabbing, burning or stinging pain.  She describes it
as an intense, painful pressure.  She did describe a possible yeast-like
diaper rash in the baby, but could see no evidence of thrush.  Still, I
referred her to her practitioner for yeast treatment as she denied
everything else I thought it might be.  Anyway, her medical practitioner
treated her with oral diflucan for three days and told her she thought she
had a plugged duct in her nipple although neither of them could feel a lump.
 She was told the baby didn't need to be treated as no thrush was seen in
his mouth so he could not have a yeast infection.  The diaper rash was
determined not to be yeast.
  Anyway, I called yesterday and mom said things had seemed to get better
for a few days, but the pain was coming back.  Said she's tried massage and
different positions to release a possible plugged duct, but feels no lump,
even now.  She added at this time that the pain tended to be concentrated in
one spot at the bottom of the areola of her right nipple. Interestingly, she
said holding a warm rag on the spot while nursing was a HUGE relief for a
while.  She stopped a couple of days ago as the pain went away.
  She's anxious now that the pain is coming back, says it made her cry at
every feeding.  I have her permission to post.  Am I missing something?  Is
it worth it to still treat for yeast (assuming I cdould convince her
practitioner).  I'm stumped and would appreciate any advice.
