We chart on the Newborn's chart.  We want the Pediatrician to be able to read
what is happening at the breast, since the baby is his/her patient.  Years
ago one of the LC's kept charting on the Mom.  She felt it was Mom she was
working with and that is where she should chart.  We had to "overrule" her and we
all have charted on the babies charts for several years now.  We chart on Mom
if the baby is in the NICU.  And we chart in NICU when we are in there helping
Mom and baby.  We chart on the feeding chart (LATCH) if we assess a feeding.
We chart on the educational record, what we taught and to whom.  And we chart
any long notes that might be needed if there were issues we need to go into
detail on the Interdisciplinary notes.  We haven't gone to computerized
charting, yet.  It is up and coming.
