I have been out of town so I haven't followed this thread. Excuse me if I am
repeating information. Yes, there is deliberated overhydration prior to
epidural administration (of about a liter of fluid) to help prevent
hypotension. Pitocin also causes the body to hang on to the fluid however
the fluid load of pitocin alone is usually a small amt. Pitocin has a very
short half life but I don't know how long the fluid retention remains. A
fluid bolus is also given when a disconcerting deceleration is noted on the
monitor and the baby is suspected to be in trouble. I can't say how often
this may occur but sometimes the Mom does seem to get more fluid than she
needs especially when a labor goes on for a very long time. There has been
more concern for the overhydration of laboring mothers and my institution has
developed a new policy regarding hydration in laboring moms. This was as a
result of concern for possible complications outside of breastfeeding. I
think it is interesting as treatment for a woman w/ pre-term contractions is
a fluid bolus. Could the overhydration impede the normal labor process?
Comments? Of course, the fluid won't stop true labor if it is going to
happen. Anyhow, it is food for thought.
Regards, Suzanne Bowes RN, IBCLC (I am sometime disguised as a labor nurse)
