yes, there is probable more to clip.  But you may not convince the ENT of
that.

Have you tried a nipple shield?  If it is the strong flow that cares baby
and he gets more shallow on purpose, it can help to break the spray and the
milk pools in the tip so baby can take it when he is ready.

But if the problem is that the tongue cannot lift enough to remove milk
well, shield may help with nipple pain but not milk removal.
