As someone who has worked with midwives and taught many in NSW Australia to assist women to learn how to feed their babies, I rarely see a midwife /child and family health nurse / neonatal intensive care nurse  teaching a woman the transition (twin) hold as the first method of feeding. 

I have had nearly 2000 staff through my basic four day breastfeeding workshop in 20 years and it would be crazy to teach anyone to use the transition hold first. Even pillows are a bit archaic. The role of the pillow in my teaching is to support the woman's elbow/arm once the baby is properly attached (ie acting as the arm of an armchair).

As a long standing IBCLC I place an enormous emphasis on not putting other professional groups or peer support groups down. It is too easy to say they did this or they did that. The focus should be on the fact that the person was working to improve things for the woman. At times, lack of experience or insight may lead to problems, but it is vital that we support each other and not put people down. I would rarely say what someone else had taught was wrong, I usually say "Did it work?" Rarely incorrect or dated practices do work so I then respond "they must have tried everything else first with you because as you have found it rarely helps".
