Totally shooting from the hip and probably WAAAY
OUT in left field:  tongue tie ?

Some types of tongue ties interfere with the baby’s ability
to handle a bolus of food.

As a LLLLeader I missed a TT because mama didn’t
complain about pain and baby (2nd baby) gained weight at a normal
pace. Mama came back to the meeting when son was almost
2yrs old asking for a frenotomy referral because son was
having difficulty with speech.  Toddler opened his mouth and I fell to
the floor !! (figuratively) 

I’ve also had a situation where a 9mo wouldn’t handle
any food either; mom took baby back to practitioner who performed
the first TT revision and after listening to mom they decided
to do the second revision.  Practitioner stated that  the 
revision needed to be deeper, into the muscle.

You haven’t mentioned anything about TT; but might this may be worth
looking at from a different angle ?
