Could someone (anyone ) help me with this situation?

I saw a mom today in clinic for baby's 2 month immunization. Nice fat
breastfed baby over 95th percentile for wt ( actually, now that I'm
thinking about it, it was off the scale for wt! and over 95th percentile
for length.) Totally breastfed, no supplements, no solids. In inquiring
how the breastfeeding was going, mom indicated she gets an occassional
sharp pain in one breast near or after completion of feeding. There were
no signs of oral thrush, no diaper rash on baby, no past history of
antiobiotic treatment for mom, or hx of yeast infections in mom. What
would be causing this pain? She did mention she did have cracked nipple on
this breast in the early initiation stages of breastfeeding. Being I was
immunizing I did not have the luxuery of assessing the latch or condition
of nipples but mother reassured me nipples were normal ie no reddness, no
flaking etc.and that latch felt good. Is this possible thrush? What would
you advise for this mom? What other questions could or should have been
looked at? Thanks for the input.
