Hello Cian - 

With crescent shaped cracks, it is highly unlikely that baby is well latched.  There also may be a secondary infection from the wounds.  Treating the wounds is the province of a physician but if the latch is not fixed, the wounds will probably return.

It is possible that the reluctance to pump has to do with modesty.  I am finding that modesty is a very big issue here.

You are right that most women in India seem to nurse for the first months and I have seen quite a few toddlers nursing since I moved to southern India in April.  Unfortunately, I have found neither IBCLCs nor LLL type groups here.  Information from the medical community (all second hand to me) seems to be comparable to what was in the US.  Some good, some fair and some...
well, you just have to wonder if they know milk comes from breasts and is OK for babies to consume...
