Hi,

I have a mom with an 8-month old and with pretty severe nipple pain in her left breast. This pain began 12 days ago, and she describes it as a "hot poker", but not burning. The pain is internal, and was preceded by a bleb (which she said "dissolved") and a caked area in the 3:00-6:00 quadrant of the breast. This caked area has resolved, but the pain persists. It was at a level of 9, now she describes it as a 6. Mom says the nipple hurts for an entire hour after feedings end. 

She has been to her OB GYN who put her on antibiotics. 10 days ago, and the mom has finished these. They also have her using a topical  calendula cream.She was in my office today, and I identified a tooth erupting on the baby's lower gumline, which mother was not aware of. I am not sure if there is a connection between this and the ongoing pain. Do you think that there is a conncection, and if so, how would this connect the dots? I also observed baby nursing - he is a typcial 8-month old and pulls away from the breast frequently to look around. I did not note any triphasic color changes indicitave of vasospasm/Raynauds.

As for resolving the problem, I have her now soaking the breasts in epsom salts prior to nursing, and trying to hand express after nursing to try to express out any plug that may exist deep under the nipple tissue. I have also referred her to a local practitioner who has experience with ultrasound on plugged ducts.

Please let me know if I am missing anything here!! I would appreciate any other suggestions from those on the list.
