Hi all, I am posting a case as requested by director of  Women's
Hospital,Indianapolis Lactation Center:
For Maryann Monaghan,RN,IBCLC:

We have the mom to an 8 month old complaining of burning,deep breast pain
only in one side after feeds.Baby did have a diaper rash but mom had no s/s
thrush on breasts....yet mom was treated for 3 weeks with nystatin cream
and baby was treated for thrush( p.o.?)
Both mom and babe were on antibx. prior to the advent of symptoms . The 3
week regimen improved diaper rash but no change or improvement in mom's
pain. Mother now has a milk blister on the painful side's nipple,increasing
pain-it appears after feeds.There is no use of tight or underwire bra and
no s/s plugged ducts.Due to the milk blister and burning pain on the one
side,mom is pumping that side this week and continuing nursing on opposite
side. Any questions or suggestions are welcome.Does thrush occur
unilaterally?There is a question of compliance with regards to yeast
avoidance measures being followed by patient.The patient has been seen by a
breast-friendly breast specialist(Dr Schmidt).We appreciate any input you
can offer.
