Emily, before treating (feeding protocol, supplementation, etc) we need to make a diagnosis. 
How and how often the baby is baby fed right now? What is on the physical exam?
Past history - born on time? Any problems during pregnancy, labor/delivery, the first days of life?
What was happening in the past 3 months?
When did the baby started to lose weight?
Any other issues (health, development, sensory?)
What were the results of the newborn screening? Was more metabolic testing done recently? 
Any problems on the cardiac exam?
