For the first time since becoming an IBCLC, so did I. A mother from a local
childbirth class (where I taught the "first days" class) called. Her baby
was born by cesarean (d/t transverse positioning).

By 24 hours, the baby was being given Similac because her "milk had not come
in." He was in a bili-blanket because "The doctor told me his bili level was
at a dangerous level." (but she doesn't know what the level is...) Yet, when
our friend visited them, she said he was pink, healthy and had APGARS of
9/10 at birth.

I suggested s2s, she said the doctor told her he had to be in the
bili-blanket. I suggested frequent nursing, and giving tiny bits of formula
if absolutely necessary, he told her she had to give the entire 2 ounces
because of his blood glucose levels (which were at 40). Baby is stooling
well, and has no symptoms of low blood sugar.

And all of a sudden, it hit me. I am contradicting a HCP! This is a new
pediatrician in the area, and the feeling I'm getting is not very good.

I gave her the AAP information about jaundice/bili levels, reminded her of
the benefits of s2s, reminded her that babies cannot regulate their
temperatures and low temps are in direct connection with low blood glucose.

Got off the phone shaking. It's like I'm being a secret spy in the camp,
trying to feed the right information, with nothing to back me up. I think I
preferred being a LLL to this.
