Molly - I would like to reply as a mom about to have a baby - I think your 
policy of a consent form is a gold standard, and should be viewed in that 
light rather than "what other hospitals are doing";  I would feel very 
comforted knowing the hospital I was delivering at took my decision to 
breastfeed so seriously - one of my biggest fears is a well-meaning yet 
misinformed nurse slipping a little formula to my baby to "help me rest", 
which is basically what happened with my first son.  Risk management should 
understand that to NOT have this consent form in place sets them up for 
being sued if a breastfeeding mom finds out her child received a 
supplemental feeding without her consent -  Good luck!!
