It is a confusing picture being painted by this baby - seemingly adequate
output, yet no weight gain.
I'd second Joy's ideas on this and ask you to keep a look out for the rare,
but possible, causes such as malabsorption, or cardiac problems, etc,
etc.  A check up with the paediatrician wouldn't go astray.
But in the meantime I would then take today (ie your first time seeing this
baby) as your starting point.  Assessing and correcting latch, assessing if
this baby urgently needs complements, is mother able to pump enough to
provide them or is donor milk or formula necessary?  Then review baby on a
secondly daily basis, with follow up phone calls on the intervening day
until you see weight gain.
I'd use everything in my 'bag of tricks' to get this mother's milk supply
overflowing and give pumped milk (preferably at the breast) but anyway I
could until weight was going up.
With any luck at all with your correction of positioning and latch
improving suck, and giving extra milk for extra energy this baby will soon
be on the road to recovery - but he will need very close followup.
Why did it happen?  Excluding organic reasons possibly output has just
gradually been going down and not that noticeable to the parents as the
initial oversupply of lactogenesis II settles down.  Baby has been
surviving on a good milk ejection from a good supply but that's just not
been enough to grow well on.
