In describing the "sandwich" approach I ask the parents to think
about how they approach a B..M.. or W....er (thick sandwiches
available at popular fast food places).  Most people tilt the
sandwich upward and put their lower jaw on first, then "fold" the
sandwich into their mouth.  Another example is how most people first
bite into a large apple with the lower teeth biting in first, then
the apple is tilted into the mouth and the upper teeth complete the
bite.

I more commonly have fallen into using my hand to demonstrate.  I
will try to describe this.  I make a "soft" fist with the middle
finger's knuckle extending further than the rest.  The hand becomes
the breast and the partially extended knuckle, the nipple.  I then
demonstrate with my mouth how to aim the nipple upward, get the
lower jaw well onto the "breast" and then bring the rest of the
nipple/areola in over the lower jaw and tongue.  (It's hard to
describe, much easier to demonstrate).
