I would say that a large nipple 'can' cause problems with a small baby ~(but
i'm sure it improves with time and babies growth).  How about trying an
'asymetrical latch'?

Another thing I wondered from your comment - "pain with latchon which stops
when I applied pressure to the baby's chin",  is the babies tongue forward?
applying pressure to the tongue usually brings the tongue forward, if it is
behind the ridge that too could be helping to fill the oral cavity as well.

Can extra long and very large nipple fill up the oral cavity so much that it
inhibits milk flow?

Mom is experiencing
pain with latchon which stops when I applied pressure to the baby's chin.
Mom has very long and large nipple.  This baby is full term and normal
weight.  Any thoughts on size of nipple causing the problem?
