I know you did mention that this mother had been worked up for everything
... but I wonder if they checked thyroid function.  Postpartum thyroiditis
could present with a low grade fever (being metric I haven't a clue if 101
is low grade or not).  These women tend to have a good supply and heaps of
energy, but as the condition progresses from hyperthyroidism to
hypothyroidism the milk supply (and the energy) dwindle accordingly.
