I hate to bring this up, but we faced this situation a few years ago.  After much discussion with the mother, she told us she had pumped after drinking alcohol (knew she shouldn't have done it but realized she didn't want to lose her supply; in her confusion, she failed to discard the first milk pumped after drinking - bingo.)  I don't know if there are other reasons milk would fail to freeze, but I am curious to hear the responses.
