Regarding the discussion of nipple surgery:

American Medical Women's Association, Vol 20, No. 10, pp 931-935.

Describes the surgical correction of "deformed" nipples.  Briefly covers
the "genetics" of "deformed" nipples, history of methods of correction,
surgical correction with caution given to avoid injuring the ducts so as
not to disturb lactation.  In results discussion, mentions that 5 of the 8
women in the trial went on to conceive, carry normal pregnancy and 4 of
these 5 went on to breastfeed.  Does not discuss their lactation in detail.
 His referrences were European journals.
