Dear Lactnetters,
        I have been working with a woman with PCOS who has delivered her
second baby and was successfully breastfeeding thanks to the help of a
hospital-based LC who worked closely with us and intervened immediately,
hyperstimulation with a Medela Symphony pump, and reglan.  Yesterday (3 weeks
postpartum) she called to resport a decline in her breast milk and admitted
to depression.  She had decreased the dosage of reglan from QID to tid two
days prior and had stopped pumping.  Baby has been nursing well every 3 hours
round the clock.

       Recognizing that the depression could be related  to the reglan, I
suggested switching to Domperidone and she agreed.  Her physician wants her
to discontinue the reglan immediately and she refuses.  She Has ordered
Domperidone and wants to continue for a few more days until she receives it.

Here are my questions:

1.  Has anyone worked with a mom with PCOS, on reglan or a galactogogue,
where the milk supply diminished despite a healthy baby nursing round the
clock?

2.  If she discontinues reglan before taking domperidine, will the milk
supply diminish and not be recoverable with domperidone?

3.  Is there any protocol for stopping reglan and starting domperidone?
