You sound soooo thorough.  I'd love to see one of your 7 page reports.  :)   I try to keep mine limited to one page.  It's hard...sometimes they go longer.  A subsequent report might be longer, but I think it has a better chance of being read if it's short.  I also tend to categorize thoughts in bullets.  Also, and what I consider to be most important, is I write what I am observing AND what I'm looking for.  My hope is they will learn what normal is from my tiny report and refer accordingly.  A short report, to me, shows that I am respectful of their time.
The plus side of this style of report is that often the doctor will call me for other details.  Not that I left anything off the report, but I think it piques their interest and leaves them wanting to learn more.  Anyway, that's what works for me.
