Actually, if we look closely to the WHO growth standards a child this size ideally would gain 30-40grams a day after regaining birthweight by 72 hours. See <http://www.who.int/childgrowth/standards/weight_for_age/en/index.html> and sroll down to the section ''expanded tables for the construction of national health cards''. When comparing the growth between percentiles it seems that the daily growth differs from 0.8% of current weight for big-born babies to 1% or a bit more for small-born babies. This will stay for about the first month of life. 
Of course the way to achieve this is to keep mom and baby in close proximity with lots of direct skin contact and frequent small feeds.

The  Paed told her to supplement after every breast feed, as he expected a large baby 4.200 kg baby to put on at least 70 grams a day.  Grrr.  I  teach that  20- 30 grams a day is a normal weight gain.
 So now this woman's breastfeeding partnership is nearly ruined, by a
misinformed paed.
Any comments?
