I agree and it is actually my policy never to give a shield until I see
clear signs that the milk is coming in. In my experience they just don't
work prior to lactogenesis. At 12 hours it is likely that baby will suck
and suck and get nothing or next to nothing. I've seen this whenever (in
my early practice days) I tried a shield early on.

I would personally rather see mom pumped and baby supplemented by some
other way.  Of course, at 12 hours.....whats wrong with a little manual
expression into baby's mouth?

Has anyone out there had success with early shield use? I'd like to hear
about it.
