I like to suggest visualization for painful letdowns.  Over the years I've
had moms tell me about painful letdowns -- some of them started after a
breastfeeding acquaintance, friend or relative told them "a let-down feels
like --------" (fill in the blank - an electric shock, pins and needles...)

I remember working with one mom with a particularly painful letdown -- she
began visualizing her letdowns as a creamy cone dispenser - slow, steady,
mom-friendly.  None of this pain visualization.  Within twenty four hours
she was feeeling considerably improved, and went on to nurse for many months
more.

Can't hurt -- worth a try if mom is willing!
