I do have the slide in question.  It was given to me when I did a lecture 
either in Idaho or somewhere in rural California over 10 years ago.  I am 
embarrassed that I cannot find the original slide on which I gave the attribution.  
The slide on the web site quoted is my slide, but no-one asked my permission to 
use it.  I would be happy to share the slide with anyone.  I will endeavor to 
find the originator, but I do have permission to share the slide at will.
