Hi, 
I don't post very often, but I have run into a couple of situations that I
thought I could use some help on.  First, I have recently worked with a
couple of mothers with low milk supply.  We have written up care plans, and
carefully reviewed the elements of building a supply.  However, on the
follow-up calls I get the feeling that even though they "say" they are
following the care plan, they "really" are not.  The milk supply is as bad
as ever, and nothing seems to be improving it.  (Of course, I know that
this sometimes truly happens, but this is a feeling that I am getting by
the vague information and other cues that I am hearing from the mother). 
Do many of you also run into the situation?  Is the mother trying to
sabotage the breastfeeding, or is it just as likely that she is really not
aware of what is really happening in her life because of sleep deprivation
etc?  Do you keep pushing, or step back some in these situations?  Is there
any good way to find out sensitively what is going on? 
Second, there is a mother here with a low milk supply, who was told by her
mid-wife that it is because "her breasts are completely blocked".  The
midwife is performing massage every day, that the mother is reporting as
very painful (during and for several hours afterward).  There are other
things going on here that would normally lead to a low milk supply.  I have
never heard of this, however, would be interested in any further
information.
