Permission to post from mama:

So i am working with a mom, in my LLLL role. (side note, I am a newly certified IBCLC in the process of opening a private practice, but haven't done all the necessary legal stuff yet...LLC, etc so I saw her under my LLL role, not the IBCLC role....fwiw) Anyway, mom has a 9mo baby girl. Was just diagnosed a few weeks ago with a posterior tongue tie after 9mo of nipple pain...eek. They had a laser frenotomy done about a week or two ago. Just had it checked again today to make sure there was no reattachment and dentist said all looks great. Mom is still having c/o severe nipple pain and damage. Lipstick shaped nipples after feeds. I have done tons of latch education for her, but baby just can't seem to get it. Per mom, if baby opens wide and latches well, after a few times she narrows her jaw and moves on with a very vigorous and painful suck almost biting on the nipple. They also did CST this week. Mom says, even after the frenotomy baby does not stick out tongue. What is the next step?? Are there SLP or OT specialist that work on tongue function? BTW...she already tried a nipple shield and really hated it so quit using it.

I keep enforcing proper latch work. Mom's nipples are very painful. Any suggestions or resources?? TIA

Emily, RN, BSN, IBCLC, LLLL
