Hello friends,
I did a lactnet search and found a whole interesting discussion on this topic but nothing practical to tell this mother.  Perhaps someone can help out there.  
I first worked with the mother when the baby was 7 days old.  The mother was experiencing pain during feeds and nipples were damaged.  The mother has a history of painful breastfeeding.  This is 5th baby.  The baby's frenulum looked a bit tight.  Mother said that one of her other children had been mildly tongue tied.  With very careful positioning she was able to nurse relatively comfortably and baby came off the breast satisfied.  Follow up phone calls were positive.  Now, a month after birth, mother says that she still experiences pain when the baby starts to feed.  It doesn't happen at all feeds and can last anywhere from a few seconds to  5 minutes.  She doesn't see any indications of what causes it.  For instance, baby's latch can look perfect to her and it will hurt, milk will be flowing before she latches baby on and it may hurt.  Sometimes - most of the time - it hurts and sometimes it doesn't.  Nipples are no longer damaged.
Anything I can suggest to this mother to ease the pain or at least explain it?  If the problem is that  tongue movement is slightly limited, will this pass with time?
Any advice would be greatly appreciated.
