Kirsten,
There is no research on long-term domperidone usage for lactation. All we
have is anecdotal info, and there are quite a few mothers out there who have
used it long-term and been fine. One thing I do hear is that they may gain
weight (not sure if it's the drug itself, or an increase in appetite caused
by the drug), and some women (generally on higher dosages) may experience
headaches. 

The reason for staying on the medication is not necessarily psychological.
There are quite a number of women who seem to have some kind of primary
problem from the very start. Domperidone seems to "super-charge" whatever
milk making cells they do have and get the most out of them. In a normal
situation where supply dropped due to mismanagement, or when milk was just
slow to come in, moms can be weaned off of the medication. In other
situations, there body just doesn't produce milk well without the
assistance. They can try to wean slowly, but still they often find a point
where it just drops to much. 

While we don't have research on lactation and long-term use, there *is*
research on longer use, and also on higher dosage use, in the
gastrointestinal application literature. If you do a search on domperidone
without lactation/breastfeeding, you'll find this info.
