They don't directly cause flat nipples, but what I have seen is
areolar edema secondary to lots of IV fluids and they really load
mom up on the IV fluids with epidurals.  Perhaps there is a more
direct connection with the epidural drugs rather than the amount of
fluid, but I think it is still areolar edema rather than actually
flat nipples.  See the archives regarding RPS (Reverse Pressure
Softening) for how best to deal with this temporary (but hitting at
the worst possible time) complication.
