I am enticed out of lurking to respond to the nipple shield posting.  Anne
Garrett at ILCA described the shield application method I have used for years
with much better stimulus of MER and milk delivery to infant.  We are in the
process of writing thi up and adding visuals and will have it available for
those interested. Format to be determined.  We use shields with discrimination,
warnings and careful followup and find they have a valued place in lactation
management.
BRIEF intro: started with Nursing Mother's Counsel 18 years ago, went to nursing
school, some private practice as IBCLC - At Stanford for fifteen years, started
lactation center here 5 years ago - which has gone from part time to a staff of
four: we see inpts., outpts from Stanford and the moms and babies needing
consults at the children's hospital next door.  Prenatal and postpartum
breastfeeding classes, continuing ed programs for nurses, elective rotations for
residents in the lactation center, pump rooms in the hospital and annex for
working nursing moms employees.
  Lots of work to do,  room for improvement.  Love it.  Am interested in
discussion of clinical approaches, new trends, prophylactic care plans (we need
one for thrush, antibiotic use is so high with sepsis protocols, high risk
babies, etc.)
Want to know if you are seeing fluid retention secondary to Pitocin? IV
infusions with epidurals? and management of areola/nipple edema.
Appreciate forum and input from you all.
