Have you tried a SNS or syringe/feeding tube at the breast? I've had good
results with getting some instant flow to baby by tucking the tube under the
nipple shield and letting the milk trickle in-- may keep baby's interest
longer and help get things going.

This is also a good way to get that "extra" fortified milk (that the MD is
certain most premies need) into babies without doing it by bottles.
