I wonder if you could say something like that it is not medically ethical to put a baby through a procedure such as circumcision, without providing any form of pain relief.  Tell her that she has the option of being present during the procedure and having her baby suck on her finger or something, if she doesn't want the pacifier used.

Mothers need to know that research, as well as common sense, indicates that feeding behaviors tend to break down in the first 24-36 hours after circumcision and can contribute to breastfeeding failure.  Another thing mothers may not understand is that it is not like pulling out a splinter, where once it is over it stops hurting.  They need to understand that circumcision leaves a wound that takes a while to heal.  A mother confused on that issue would probably not have thought about the fact that any anesthesia that is used will wear off well before the penis is healed enough to stop hurting, either.
