Sherry writes about a mom with nipple soreness/trauma on upper nipple face 
due to her baby's tooth.  Cathy Genna posted awhile ago about moms getting a 
very asymmetrical latch so the breast is in less contact with upper teeth.  I 
know you said mom has tried different positioning but perhaps she can try to 
alter latch by just aligning baby nipple to nose, with head slightly extended.
