The first thing that comes to mind is something in mom's diet that is showing up at that time of day. but I am sure you have already explored that. The other thing that comes to mind is lactose overload, which doesnt' seem to make sense for the end of the day, but possibly the feeding pattern is large consumption at some part of the day? or does baby take any pumped milk or is in the care of any other person during any part of the 24 hr day? possibly consuming other milk or solids unbeknownst to mother?
If the baby checks out healthy by the primary care provider, perhaps mother can explore taking digestive enzymes herself and/or probiotics/prebiotics for baby? Just brainstorming.
