MK,
The secret around JCAHO.....they expect the P&Ps to be reviewed and so you
review them, update them if necessary, according to the latest evidence, and
change the revision date and you're okay.  If no changes are necessary, then you
type the new date of the revision/review on the policy and you should be set
to go!
