Hi all-
Here's a case I would like some input on:
Mom has a 19 month old, whom she breastfed until 10 months of age. He is
totally weaned since that time, yet she continues to have plenty of milk, to
the point of leaking a lot each morning, and sometimes is full enough that
she will express some for comfort, tho not always. She describes herself as
"terribly fertile", and she and husband want no more children (have 2), so
she is TRYING not to get pregnant. She was given DepoProvera at the health
clinic, and now presents with these complaints: Continued milk abundance; a
lump in the right upper quadrant of R breast that "seems to come and go", is
mildly painful at times; no menses at all since the last child's birth; and a
13 pound weight LOSS  in 3 weeks. She appears to be a relatively anxious
person, but this may also be due to how much all this is currently worrying
her. Other medical hx includes gastric ulcer, hiatal hernia, family hx of
fibrocystic breast disease and several spontaneous ABs, including one
multiple gestation.
1) I know DP can increase milk supply in some-is this a possiblilty?
2) the lump may be a plugged duct intermittently, or cystic.
3) the rapid weight loss is alarming

I referred this mom to a private gynecologist based on the above. She had
gone to the public health clinic for prental care because her husbands
insurance was not in effect when she became pregnant, but now is covered and
can seek private care. The clinic practitioner has not addressed these
complaints to her satisfaction (nor mine).
