OK, there is an out.

HCPs must document how infant is to  be fed.  If mom choses formula then HCP 
must document "mother's choice" or maternal illness or infant situation. 
There are a number of HCPs listed (including LCs-which are last choice.)

Your hospital policy can be that Dr, CNM , APN or PA can document choice - 
breast or formula(and reason).  Feeding method  is really the job of the 
admitting HCP.

Then if a breastfed baby is supplemented any HCP can document reason.
