Hi Judy -
You said you are looking for options other than galactagogues. Not using
them is a good idea anyway. I have not found them to be particularly
helpful in the situation that you describe anyway. Mom is now at a point
where milk removal governs production. Your description looks like she is
only stimulating her breasts about 5 times per day, only twice with a baby.

In this type of situation, I would encourage mom to work toward at least 8
stimulations per day. As many of these stimulations should be by baby as
possible.

In addition, if baby is going to get cereal, let the sitter give it in mom's
absence. This will do two things for mom and baby. First, it will reduce
the amount of food (milk) needed when mom is absent. Second, not using
cereal when mom is available will increase the amount of milk baby needs to
eat when with mom and should increase her supply.

You might want to evaluate the pump as well. Often mom's do not clean them
properly and that will interfere with effective milk removal. It is
possible that the pump might be defective or inadequate for this mom or she
may need different size flanges.

Good luck with this situation.
