I have a couple of thoughts on this. Have cultures been done(on mom's milk and perhaps baby too) to know exactly what they're dealing with? Then you'd know the right course of antibiotic to address the problem. My other thought is if mom has been treated twice for mastitis in the same area and she's dealing with it again, she needs to be seen by her doctor. It might be breast cancer that's presenting itself as mastitis. Sadly, I've seen that in 2 women over the course of my LC career. So, I usually say two times of trying to clear the infection up, then it's time to culture and see the Dr. I didn't see the original post, so I apologize if these things were already addressed. I hope it helps. Sue Manore, LLLL, IBCLC
